Life Insurance Terms, Underwriting Disclosures & Authorizations

Ascension Financial — Group Life Insurance Disclosures & Authorizations

1. The benefit. One World Fund ("OWF") has arranged access to a group term life insurance benefit underwritten and administered by Ascension Financial ("the Insurer"). This coverage is an insurance product provided by the Insurer. It is separate from, and in addition to, your OWF mutual aid membership, which is not insurance.

2. Eligibility. Coverage is offered to active OWF members who meet the Insurer's eligibility requirements (generally ages 18–70). Final eligibility, coverage amounts, effective dates, exclusions, and premiums (if any) are determined solely by the Insurer and will be set out in the policy or certificate of coverage the Insurer issues to you.

3. Authorization to share your information. By selecting "Yes, activate my coverage," you authorize OWF to disclose the registration information listed below to Ascension Financial for the purpose of evaluating your eligibility, underwriting, issuing, and administering your policy: your first and last name, email address, Social Security Number, mailing address, beneficiary information, and membership sign-up (join) date.

4. Underwriting and verification authorization. You authorize Ascension Financial, and its authorized service providers, to collect, use, and verify the information above (and any additional information you provide directly to the Insurer, which may include health or medical information where required) to the extent necessary to underwrite and administer your policy, consistent with applicable law. Any medical or health information you submit is provided to and held by the Insurer under its own privacy practices.

5. Electronic delivery & records. You consent to receive insurance-related disclosures, the policy or certificate, and other communications electronically. You may request paper copies or withdraw this consent by contacting OWF support or the Insurer.

6. No guarantee; voluntary. Enrollment is entirely voluntary. Declining coverage has no effect on your OWF membership or benefits. OWF is not the insurer, does not underwrite this coverage, and does not guarantee approval, coverage amounts, or claim outcomes, all of which are the responsibility of Ascension Financial.

7. Your rights. You may decline now or opt out later, and you may revoke this authorization on a going-forward basis, by contacting OWF support at info@oneworldfund.life. Revocation does not affect information already shared or actions already taken in reliance on your authorization. Your information is handled in accordance with the OWF Privacy Policy and the Insurer's privacy practices.
